On page 17 unde...actually, that's a good catch. On page 17 under "initiative," it has the 2d6+HP rule (which I apparently missed the first read-through), but on page 16 under "turns," it has the all-players-act-simultaneously rule. simultaneous actions make more sense in non-combat situations where time is still nevertheless a factor (but honestly I'd want to factor injury in to that as well), but initiative order makes more sense in combat.
